Diabetes Management Program

Valley Health’s Diabetes Management Program can answer the many questions that come with the diagnosis of Diabetes. Questions like, what does this mean? What can I eat? What other problems do I face? The management staff consists of registered nurses, registered dietitians, health educators and insulin pump clinical nurse specialists; all ready to help patients make the lifestyle changes necessary to improve health and quality of life. Diabetes can be successfully managed through education, goal setting and life style and behavioral changes.

The Diabetes Management Program offers services including:

A physician’s referral is required for the Diabetes Management Program, excluding pre-diabetes education and Eating Health
